PREPARE for Home
PREPARE has a vibrant and expanding network of reentry partners and community resources. The PREPARE for Home phase of our program offers individualized reentry planning services to incarcerated people starting seven months before their parole hearing. We also assist individuals approaching mandatory release.
PREPARE for Home services include:
- A PREPARE for Home intake and Reentry Needs Assessment, which identifies the needs of the client, specific to their desired county
- A custom reentry plan, created by a reentry specialist based on the client’s needs. PREPARE has a vast network of resources to meet specific client needs (mental health, substance abuse, transitional housing and employment are a few examples)
- A list of service providers that match their needs. When feasible, the reentry specialist helps arrange interviews and receive acceptance or eligibility letters
- A copy of eligibility letters and documentation will be provided to Case Management, Reentry Services, and the Parole Commission to demonstrate the client has a thoughtful plan
